Vital Statistics Services Provided by New Philadelphia City Health Department

NPCHD provides many vital statisitics services. These include:

• Certified Death Certificates for deaths that have occured in New Philadelphia
• Certified Birth Certificates for anyone born in the state of Ohio after 1908
• Burial and Cremation Permits
• E-filing of "Death Records" is now available using the Request for Approval for Electronic Death Certificate Form and emailing the form and the certificate to vitalstats@newphilaoh.com. 

Placing An Order:

For the fastest response, it is recommended to place your order in person. You can call our office at 330-364‐4491, extension 1208 for detailed instructions.

You can download the application for Birth and Death Certificates using the link under Downloadable Documents.  Please complete one application form for each record or search requested.  

Fees:

In accordance with section 3705.24 of the Ohio Revised Code, NPCHD is required by law to charge a fee for each certified copy of a vital record issued. The fee at this office for each certified copy of a birth, death, or fetal death record is $25.00.

The fee for burial and cremation permits is $3.00.
